what is supported home living?
Helping People Thrive in the Comfort of Home
Supported Home Living helps individuals with intellectual and developmental disabilities remain active and connected in their own home, family home, and community.
Under HCS, Supported Home Living may include *transportation* when it is identified as a need in the person-directed plan. Transportation can support approved goals such as getting to work, appointments, shopping, or other community activities. ([Texas Health and Human Services])
For assistance with personal care, daily living activities, and habilitation in the home, individuals may also receive Community First Choice Personal Assistance Services/Habilitation (CFC PAS/HAB) when included in their approved plan. ([Texas Health and Human Services])
Support for Independent Living
Living in your own home or family home can provide comfort, stability, and familiarity. Some individuals may also need additional support to maintain their independence and participate fully in everyday life.
Depending on the individual’s approved HCS plan, support may include personal assistance, help with daily routines, skill development, and transportation connected to identified goals and needs.
At NUDO Homes, we work with each individual and their support team to provide services according to the person’s needs, preferences, and approved plan.
How it works
The support you need for independent living
For many individuals with intellectual and developmental disabilities (IDD), remaining in their familiar home environment can provide greater comfort, stability, and security. Staying at home may also help minimize the challenges that can come with moving to a new environment. However, additional support may be necessary to make living at home possible.
Our Supported Home Living Services are designed to help individuals with IDD live as independently as possible in their own homes. We provide assistance with everyday activities, including eating, bathing, dressing, grooming, and personal hygiene.
Since every individual has different needs, preferences, and abilities, the type and level of support provided will vary. Each Supported Home Living plan is personalized to ensure the individual receives the appropriate assistance while maintaining dignity, independence, and quality of life.
