WHAT IS HOST HOME/COMPANION CARE?
A Supportive Home Environment Built Around the Individual
Host Home/Companion Care is an HCS residential service for individuals with intellectual and developmental disabilities who benefit from living in a smaller, home-based setting with ongoing support.
The service provider lives in the same residence and provides assistance based on the individual’s approved HCS plan and daily support needs. Support may include help with personal care, meals, household routines, mobility, medications, transportation, safety, community participation, and development of daily living skills.
How it works
Personalized Support in a Home Setting
Host Home/Companion Care is provided in a private residence where the individual lives with the person providing the service.
The difference between the two arrangements is primarily who has the property interest in the home. In a Host Home, the service provider owns or leases the residence. In Companion Care, the residence may be owned or leased by either the individual or the service provider.
No more than three individuals may receive Host Home/Companion Care or a similar service in the residence. Support is provided according to each person’s needs, preferences, abilities, and approved service plan.
The goal is to provide a stable, supportive home environment where the individual can build daily living skills, make meaningful choices, develop relationships, and participate in community life.
