what is txhml?
Texas Home Living (TxHmL) Services
Texas Home Living is a Medicaid waiver program for people with an intellectual or developmental disability who live in their own home or with their family. Rather than moving into a residential setting, the person stays right where they are, and TxHmL brings in the supports that help them build skills, stay healthy, work, and take part in their community.
TxHmL is designed to supplement what’s already in place — family, friends, school, Texas Health Steps, community organizations — not to replace it. It’s a smaller waiver than HCS, with an annual cost limit on services, but for many families it’s the right fit and it’s often available sooner.
TxHmL services include:
- Individualized Skills and Socialization (ISS)
- Community Support (including transportation)
- Employment Assistance and Supported Employment
- Respite Services
- Nursing
- Behavioral Support
- Specialized Therapies — occupational, physical, speech, audiology, and dietary
- Dental Services
- Adaptive Aids
- Minor Home Modifications
TxHmL providers can also deliver personal assistance, habilitation, and emergency response services through Community First Choice (CFC).

TxHmL Services We Provide
We provide all of the services offered by the TxHmL program, including:
INDIVIDUALIZES SKILLS AND SOCIALIZATION (ISS)
ISS helps people build and keep the skills that matter to them daily, such as living, communication, safety, and self-advocacy, through structured activities built around each person’s own goals. It can be provided in the community or at a licensed site, so the plan fits the person rather than the other way around.
COMMUNITY SUPPORT
Community Support Services help individuals develop and maintain the skills needed for greater independence at home and in the community. Depending on the individual’s assessed needs and approved plan, support may include assistance with daily routines, community participation, medication-related support, certain delegated tasks, and transportation connected to the individual’s goals and activities.
EMPLOYMENT ASSISTANCE AND SUPPORTED EMPLOYMENT
Employment Assistance helps individuals identify their employment interests, strengths, skills, and preferred work environment, and can include help locating and pursuing competitive employment in the community.
Supported Employment provides ongoing support for individuals who need assistance to maintain competitive employment. Support may include job coaching, training, workplace adaptations, and other assistance related to the individual’s assessed needs.
RESPITE SERVICES
Caring for a loved one full-time is hard work. Respite provides temporary relief for family care-givers, so you can rest, handle an emergency, or simply take a breath — with confidence that your loved one is in good hands.
NURSING
Licensed nurses provide assessments, monitoring, training, and health-related supports to help each person stay well at home.
BEHAVIORAL SUPPORT
Assessment, planning, and individualised intervention to address challenging behaviours always with the goal of improving daily life, not just managing it.
SPECIALIZED THERAPIES
Occupational therapy, physical therapy, speech therapy, audiology, and dietary services, delivered by qualified professionals.
DENTAL SERVICES
Preventive and treatment dental care, subject to the program’s annual limit.
MINOR HOME MODIFICATION
Minor home modifications are physical changes made to an individual’s residence to address specific disability-related needs and support greater safety, accessibility, independence, or control within the home.
For individuals receiving HCS services, minor home modifications may include approved adaptations that help the person function more independently in their residence.
Current HCS limits include:
Up to $7,500 in lifetime expenditures for minor home modifications
Up to $300 per IPC year for eligible repair and maintenance of approved modifications
Minor home modifications must be related to the individual’s assessed needs and approved service plan. General home repairs, remodeling, or improvements that are not directly related to the individual’s disability-related needs are not covered under this service

who qualifies
Is TxHmL right for you or your loved one?
TxHmL may be available to a Texas resident who:
- Lives in their own home or their family’s home
- Has an intellectual disability, or a related condition with an IQ of 75 or below
- Is eligible for Medicaid
- Is not enrolled in another Medicaid waiver program

TxHmL or HCS — what's the difference?
Both waivers serve people with intellectual and developmental disabilities, and both are managed by Texas HHS. The short version: HCS includes residential options like group homes and host homes/companion care and supervised living and has a broader service array with higher cost limits. TxHmL is for people who are staying in their own or family’s home and need a focused set of supports, with an annual cost limit. You can’t be in both at once.
